The Harrow Learning Disabilities Community Health Team supports adults aged 18 and over (and young people aged 17 who are transitioning from children's services) who have learning disabilities and are registered with a Harrow GP. The team consists of multi-disciplinary staff, including community nurses, psychiatrists, psychologists, and speech and language therapists. They provide assessment and treatment to people in their choice of environment, whether that be in their own home or an alternative community setting such as college, day service, supported living, residential setting, care home or hospital.
	The service offers comprehensive, holistic, specialist health assessment and intervention which aims to:
	- Support people with learning disabilities to live as valued members of society by providing and arranging specialist health care, and supporting access to generic services wherever possible.
	- Optimise safety, health and wellbeing, and to prevent avoidable admissions to assessment and treatment beds, and specialist care provision.
	- Support care closer to home, reducing the need for out of area placements and (where this is unavoidable) to facilitate return home.
	- Work in partnership with service users, their carers, advocates, IMCAs and other health and social care professionals to plan and deliver services.
